If you have had equipment provided by us, we may ask for a photograph
(where appropriate) of your child and the equipment. You are under no
obligation to provide such material.

We may also use this information to notify you
about our services or events which we believe may be of interest to you.
If you have provided an image of your child with
any equipment provided
by Equipment for Disabled Children, we may ask if we can use that image
(royalty free) for promotional purposes. We will not, however, disclose your child’s identity.

part of a request for equipment.
